Financial Information
 At the Oral and Facial Surgery Center (OFSC), we deliver the finest care at the most reasonable cost to our patients. Therefore, payment is due at the time service is rendered unless other arrangements have been made in advance.
Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.

Listed below are the payment options that we provide to all of our patients.
Payment Options
At the Oral and Facial Surgery Center, we accept checks, cash, or credit cards, and/or Care Credit. We also offer a few flexible 3rd party financing options as described below.
3rd Party Financing Options:
We understand that needed surgical procedures can be an unexpected financial burden and we are here to help! Although we do not offer in office financing options; we do have multiple 3rd party financing options that are available to you.
Option 1: BMG More MasterCard

If you wish to use your MORE Card for financing same day, you must have a credit score of 660 or higher (660 or lower can still be approved, but must wait 7-10 days for physical card to arrival via mail)

Any amount over $500 automatically includes a 6-month 0% APR with no retroactive interest.

| 4. |
Any amount under $500 receives 2% cash back.
|
| 5. |
Card can be used in any healthcare facility that accepts MasterCard. |
Option 2: Care Credit

If amount of financing is over $200, you can choose 0% APR financing options but interest will be charged on the purchase date if not paid in full within the promotional period.

Oral and Facial Surgery Center of Tallahassee only offers a 6-month 0% APR promotional period.

Option 3: Campus USA Credit Union (Personal Loan)

Applicant must live in Leon/Jackson or surrounding county |

To Apply: Call 352-335-9090 and go to loan que. You can apply and get prequalified over the phone. If you are not currently a member of the credit union, you would have to go to one of the conveniently located branch locations to open the account. You can also apply online at www.campuscu.com (Loan- apply) |
